How do I request a prescription refill?

Have your pharmacy contact our office at 770-941-4716 or fax a refill request to us at 770-941-3047.

What is the policy concerning requests for a controlled substance?

Prescriptions for narcotics (Lorcet, Hydrocodone, Vicodin, Percocet, Darvocet, etc.) and benzodiazepine drugs (Xanax, Valium, Ativan, Lorazepam) will be written at the time of your appointment only.

What if I miss my appointment?

We ask that you call us to cancel or reschedule your appointment at least 24 hours in advance. All last-minute cancellations and no-shows will incur a $25 fee.

What if I missed an appointment and need my medications refilled?

We realize that unforeseen circumstances can occur. Please contact our office and we will make every effort to get your prescription refilled.

What if I need a prescription refilled after hours?

Unfortunately, we do not call in prescriptions after hours.

How long does it take to get a prescription processed?

Our goal is to process all refill requests made before 3:00 pm on the same day they are received; this is, however, not always possible (for example, in emergencies). Requests received after 3:00 pm will be processed the next business day.

What happens if I call after hours?

Our physicians do not take routine calls after hours. If yours is an emergency situation, call 911 immediately or proceed to the emergency room nearest you.

What is your policy concerning messages?

We will try to answer any message we receive before 3:00 pm the same day if not called away on an emergency. Any message left after 3:00 pm will be answered in one to two business days.

How do I receive test results?

Test results will be discussed at your follow-up office visit.

I need my medical records—what do I do?

Click here to download a medical records request form; then fill it out and submit it to our Austell Office. You can do so in person, by mail, or by fax: 770-941-3047.

What is the charge for completion of a form?

Charges for medical forms will vary: once your doctor agrees to complete your forms, a medical assistant will advise you of what the charge will be. You should allow 7–10 business days for the completion of forms. For expedited requests—to be completed within 3 business days—there will be an additional $75 charge.
